MDS- Holistic Team-based Care Planning

Now in our 3rd year, we’re considered a “senior” in the MDS Early Adopter world!

Saugeen chose to participate in this program to improve resident care and for the opportunity to work directly with MOHLTC to provide input into mandatory long-term care standards.

The impact of MDS is widespread. MDS not only impacts resident care, MOHLTC intends to use MDS as a tool for compliance, accountability reporting, determining operational funding and to benchmark homes (creation of a public scorecard).

Saugeen Valley Nursing Center was 1 of 20 homes selected from more than 200+ applicants to pilot MDS. This experience has brought us many successes in improved resident care. We have been profiled in the Canadian Long-Term Care magazine, MOHLTC newsletters, MOHLTC roadshows/forums.

I have heard first hand the impact Saugeen has had on other Homes adopting this process and realizing the benefits to resident care and we continue be proactive with MDS.

Melinda Timmins, Nutrition Manager, is currently participating on a provincial task force that is creating a Dietary resource guide to use along with MDS to ensure we best meet the Nutritional needs of all residents. This guide will be used by all Nutrition Managers and Dietitians across Ontario.

In June, Kim McCarthy, our MDS-RAI Coordinator, attended a national MDS conference in Alberta and gained valuable insight into how we can further use MDS to enhance resident care. Plans include developing care pathways to promote assessment, streamlining processes (to reduce paperwork) and point of care charting.
