Saugeen Valley Nursing Center was selected as the first healthcare organization in Waterloo-Wellington region to participate in this initiative because of progressive attitude in promoting a resident-centered culture and our recent successes with implementing MDS.

HOBIC – Health Outcomes for Better Information and Care is a Ministry of Health initiative. HOBIC aims to produce better data, support accountability and quality improvement through performance measurement and support evidence-based decision making.

HOBIC is a series of 8 questions used to assess residents (5 questions are already answered with our current MDS assessment). All questions will be integrated into our current MDS assessment and will be completed on admission of the resident (in the first 24 hours), quarterly and if there is a significant change in the residents’ condition.

HOBIC is a provincial mandate, anticipated to be rolled out by 2009 in long-term care, acute care, complex continuing care as well as Home Care.

Since we are one of the first organizations to participate, we received special one-time funding for training & equipment, including a wireless building and 5 new wireless tablets to assist with bedside nursing & assessment.
